Electronic Forms

Electronic Forms is the option under Tools where Physician Scheduling users can complete forms.

Submit a Form

To submit a form, click on the Electronic Forms option under Tools.

This will prompt a flyover menu with a list of form options. Choose one. This action will take you to an electronic form page.

Hit when done.
